Fatty liver is common, but it should not always be considered harmless. In some people, excess fat in the liver can remain stable, while in others it may progress to liver inflammation, fibrosis, cirrhosis and, in advanced cases, liver cancer.

Today, the term MASLD (Metabolic dysfunction-associated steatotic liver disease) is increasingly used for fatty liver associated with metabolic risk factors such as obesity, diabetes, high blood pressure and abnormal cholesterol. MASH refers to the form in which liver fat is associated with inflammation and liver injury.

Fatty Liver Is Manageable

The right approach can make a meaningful difference

Finding fatty liver on an ultrasound or health check does not automatically mean that serious liver damage has occurred.

The next step is to understand:

  • How much fat is present in the liver
  • Whether there is liver inflammation
  • Whether fibrosis or scarring is present
  • Whether diabetes or prediabetes is contributing
  • Whether weight, cholesterol or blood pressure need attention
  • Whether alcohol, medicines or another condition may be contributing
  • Whether the liver needs closer monitoring

The goal is not simply to “reduce liver fat”. It is to protect liver health and reduce the risk of future liver damage.

Our Approach to Fatty Liver Management

1. Understand the Cause

Fatty liver can have several contributing factors. Metabolic health is particularly important in MASLD.

Assessment may include looking at:

  • Weight & obesity
    Excess body weight can increase the risk of liver fat accumulation.
  • Diabetes & insulin resistance
    High blood sugar and diabetes are strongly linked with fatty liver.
  • Cholesterol & triglycerides
    Abnormal blood fats can contribute to metabolic liver disease.
  • Blood pressure
    High blood pressure often occurs alongside other metabolic risk factors.
  • Alcohol intake
    Alcohol consumption needs to be considered when evaluating fatty liver.
  • Medicines and other conditions
    Some medicines and medical conditions can also contribute to liver fat.

2. Assess the Liver Properly

Fat in the liver is only one part of the picture.

A person may have significant liver disease even when they have few or no symptoms. Therefore, management should also consider the possibility of fibrosis or liver scarring.

Depending on the individual patient, assessment may include:

  • Liver Blood Tests
    To evaluate liver enzymes and other markers of liver health.
  • Ultrasound
    A commonly used imaging test that can detect fatty changes in the liver.
  • FibroScan
    A non-invasive test that can help assess liver stiffness and fat.
  • Fibrosis Risk Assessment
    Non-invasive scores and tests can help identify patients who may need further evaluation. AASLD guidance recommends assessing fibrosis risk rather than relying only on the presence of liver fat.
  • Additional Investigations
    Further tests may be recommended depending on the patient’s history, risk factors and initial findings.

2. Lifestyle Changes

The Foundation of Fatty Liver Management

Lifestyle modification remains a cornerstone of MASLD management. Diet, physical activity and sustainable weight management can help reduce liver fat and improve overall metabolic health.

Healthy Eating

There is no single “magic food” that cures fatty liver.

Instead, the focus should be on an overall healthy eating pattern that includes:

  • Vegetables and fruits
  • Whole grains
  • Pulses and legumes
  • Adequate protein
  • Nuts and seeds in appropriate portions
  • Healthy sources of unsaturated fats
  • Less highly processed food
  • Less added sugar
  • Fewer sugar-sweetened beverages

A Mediterranean-style eating pattern is one dietary approach supported by evidence for improving metabolic health and reducing liver fat.
